Rehabilitation of a maxillary premolar with subgingival fracture and unfavorable prognosis: a case report involving a multidisciplinary approach

Abstract

Introduction: The rehabilitation of teeth with severe crown destruction and biological width invasion represents a significant challenge requiring an integrated multidisciplinary approach. Clinical success depends on restoring biological distances, controlling intraradicular infection, and recovering biomechanical integrity.

Case Report: A 37-year-old female patient presented with tooth 14 exhibiting an extensive subgingival coronal fracture, interrupted endodontic treatment, and a periapical lesion. The tooth, initially indicated for extraction, underwent a sequential rehabilitative protocol that included crown lengthening surgery (CLS) associated with osteotomy to reestablish the biological width. Subsequently, deep margin elevation (DME) was performed using composite resin to transpose the restorative margin to a supragingival level. Endodontic treatment was then completed, followed by intraradicular reinforcement with a fiberglass post and core build-up. Finally, definitive prosthetic rehabilitation was performed with an indirect full crown made of high-load composite resin.

Results: The multidisciplinary strategy allowed for the overcoming of technical challenges, such as rubber dam instability in deep margins, which was resolved by refining the DME technique using copper matrices and modified clamps. Clinical and radiographic follow-up after 6 months demonstrated excellent marginal adaptation, absence of symptoms, and evident signs of periapical bone repair.

Conclusion: The synergistic integration of Periodontics, Endodontics, Restorative Dentistry, and Prosthodontics enabled the functional and esthetic maintenance of a tooth with an initially unfavorable prognosis. This case validates the effectiveness of a conservative philosophy and the importance of technical rigor in isolation and adhesive stages.
