Distribution of postpartum hemorrhage cases and associated factors in a public maternity hospital in southern Minas

Abstract

This study aimed to analyze the etiological distribution of postpartum hemorrhage and its relationship with clinical outcomes in a public maternity hospital in southern Minas Gerais. This is an observational, retrospective study with a quantitative approach and a descriptive-analytical design, conducted using secondary data from women who gave birth between January 2024 and December 2025. All cases diagnosed with postpartum hemorrhage were included, and obstetric variables and clinical outcomes were evaluated using descriptive and inferential statistical analysis. After reclassifying the cases according to the 4 Ts model, a predominance of etiology related to uterine tone was observed, accounting for 52.5% of cases, followed by a high proportion of unclassifiable cases. More severe outcomes, such as blood transfusion and admission to the intensive care unit, were more frequent among unclassifiable cases, while a higher occurrence of surgical interventions was observed in cases associated with uterine atony. It is concluded that uterine atony remains the main identifiable cause of postpartum hemorrhage; however, the high frequency of cases without a defined classification suggests limitations in medical records and a possible multifactorial nature of the condition, reinforcing the importance of standardized clinical documentation and a systematic approach to ensure proper management and reduction of maternal complications.
