Abstract

Epilepsy is the most common chronic neurological condition in the pediatric population. In most cases, it is effectively controlled with anti-seizure medications (ASMs); however, approximately one third of patients are refractory to pharmacological treatment. Neurosurgical treatment represents an alternative that increases the likelihood of improved seizure control or seizure freedom. Prior to surgery, all patients must undergo a comprehensive preoperative evaluation, ranging from neuropsychological assessment to detailed invasive investigations in selected cases. A wide variety of surgical techniques is available, and the choice depends on the individual clinical profile and the results of the preoperative workup. Although still underutilized, epilepsy surgery in the pediatric population has well-established benefits across multiple domains, including partial or complete seizure reduction as well as neurocognitive improvement. Therefore, early referral to specialized centers is recommended.
