Abstract

Stem cell therapy has been widely investigated in regenerative medicine as an alternative for recovering injured myocardium after acute infarction—a condition that causes irreversible loss of cardiomyocytes and promotes the progression toward heart failure. While conventional therapies reduce disease progression, they do not restore lost cardiac tissue, reinforcing the need for new therapeutic approaches. This study analyzes the potential of stem cells in myocardial regeneration, considering their efficacy, mechanisms of action, and main limitations. It consists of a qualitative literature review based on articles published between 2020 and 2025 in recognized scientific databases. Experimental studies and clinical trials related to cell therapy applied to the myocardium were evaluated, including discussions on regulatory and bioethical aspects. Evidence indicates that mesenchymal and induced pluripotent stem cells can reduce fibrosis, stimulate angiogenesis, and improve functional cardiac parameters. However, challenges such as low cell survival, heterogeneity in clinical protocols, and ethical issues still restrict their broad implementation. The analysis presented contributes to the understanding of the advances and limitations of this approach, providing insights for the enhancement of future therapeutic strategies.
