Comparison between the NCHS/1977 Growth Curves and the WHO/2006 Standard, in force in 2025, in the Nutritional Assessment of Children Under Two Years of Age

Monitoring child growth is one of the most sensitive tools for surveillance of health and nutritional status in early childhood. The transition from the National Center for Health Statistics (NCHS/1977) reference to the World Health Organization (WHO/2006) standard represented an important conceptual and operational change, with direct effects on anthropometric classification. In 2025, WHO reaffirmed that for children aged 0 to 5 years the officially recommended reference remains the 2006 WHO Child Growth Standards, with no new specific growth curve issued for this age group. This study aimed to compare the NCHS/1977 reference and the WHO/2006 standard in the nutritional assessment of children under two years of age attending a primary health care unit in Beneditinos, Piauí, Brazil. This was a descriptive cross-sectional study involving 116 children seen between January and July 2007. Weight-for-age and weight-for-length indices were analyzed using secondary data on weight, length and age. The findings showed a reduction in low weight/malnutrition and overweight/obesity classifications for weight-for-age when the WHO/2006 standard was applied, with an increase in eutrophy. For weight-for-length, there was a decrease in low weight and an increase in normal status, while the percentage of overweight/obesity remained unchanged. It is concluded that the adoption of the WHO/2006 standard, maintained as the current reference in 2025 for children under five years of age, changes the anthropometric distribution and tends to promote a more consistent interpretation of child growth, reinforcing its usefulness for nutritional surveillance, primary health care, and the planning of public health actions.
