Verticalização de segundo molar inferior impactado por meio de mecânica ortodôntica conservadora com fio NiTi: Relato de caso e proposta clínica baseada em fundamentos biomecânicos

Mandibular second molar impaction is a low-prevalence yet clinically significant condition that may compromise periodontal health, masticatory function, and future prosthetic rehabilitation. The literature describes multiple management approaches, ranging from conservative orthodontic mechanics to surgical uprighting with skeletal anchorage. This study aims to critically review the literature on mandibular second molar uprighting and to report a clinical case treated with a conservative orthodontic approach using a nickel-titanium wire (0.014” x 0.025” CuNiTi), with longitudinal clinical and radiographic follow-up. The patient, a 14-year-4-month-old female, presented with mesioangulated impaction of tooth 37 associated with incomplete root formation. The conservative approach promoted progressive uprighting without biological complications. Based on the clinical evolution and literature analysis, a systematic clinical pathway is proposed considering wire dimension, activation time, and stage of root development.
